He indicated that his age was 58 years, his date of birth was 27 Feb 1884, and that he was born in Memphis, [Shelby County,] Tennessee. His address was 304 Lincoln Avenue, Laclede, Missouri, and his wife Mrs. Fleda Pruitt is named as the person who would always know his address. His was employed at the time as a Carpenter.
In the Description of Registrant section, it indicates he is White, 5 feet 10 inches tall, and weighs 160 pounds. His eyes are blue, his hair is black, and complexion is sallow. There are no distinguishing marks listed.
